An inquest at Norfolk Coroner's Court heard that a blood clot had formed on the replacement valve, preventing it from working properly.

His family raised concerns about aspects of the care he received in the weeks before his death and his widow, Julie Kavanagh, described the shock of taking her husband to hospital expecting his medication to be adjusted, only for him to die two days later.

But assistant coroner Maeve Sykes found the seriousness of Mr Kavanagh’s condition had been recognised and appropriately treated.

She concluded that he had died from a recognised complication of necessary medical treatment.

Mr Kavanagh, who was born in Dublin on March 15, 1961, lived in Wootton Road, Gaywood, near King’s Lynn.

A retired police officer, he later worked as a peer support worker for Shine, helping people experiencing mental health difficulties.
