When compulsions feel protective because your brain is screaming at you that you're in danger, it makes it very challenging to engage in obsessive-compulsive disorder (OCD) treatment. Your brain can pull from traumatic experiences, creating a very convincing story that these compulsions are necessary because they feel like they are the only path to safety and certainty.
While OCD and post-traumatic stress disorder (PTSD) or complex post-traumatic stress disorder (C-PTSD) are distinct conditions with different underlying mechanisms, they frequently overlap. In fact, emerging research suggests that individuals with features of C-PTSD often experience greater OCD symptom severity than those with OCD alone (D'Angelo et al., 2024).
This does not mean trauma causes OCD. Many people experience trauma and never develop OCD, while many individuals with OCD have no significant trauma history. Like most psychological disorders, OCD develops through a combination of genetics, neurobiology, learning experiences, and environmental factors.
What trauma can do is influence how OCD is experienced. It can make intrusive thoughts feel more believable, more emotionally charged, and more difficult to disengage from. Understanding the neurobiology behind why this happens can reduce shame and help explain why treatment may feel particularly challenging. It is also important for clinicians to understand the overlap to determine the need for trauma interventions. Here are five ways OCD builds on trauma based on neurobiology.
1. Trauma sensitizes the brain's threat detection system.
One of the most replicated findings in trauma research is increased activation of the amygdala, the brain region responsible for detecting potential threat, our "fight or flight" system (Shin & Liberzon, 2010; Pitman et al., 2012). From a survival perspective, this makes perfect sense: If someone has lived through repeated danger, the brain becomes increasingly efficient at recognizing anything that might signal harm. This adaptation is extremely protective and helpful when we are in traumatic situations, especially for folks who have experienced multiple traumatic events throughout their life.
