You took your attention-deficit/hyperactivity disorder (ADHD) medication. You slept OK, and nothing disastrous happened this week.

So why are you suddenly losing your phone, rereading the same paragraph, forgetting plans, and staring at an assignment you desperately want to start?

Everything feels harder. Your usual ADHD strategies aren’t working, and even your medication doesn’t seem to be helping as much as usual.

Then your period starts.

If you’ve noticed your ADHD seems to get worse around the same time each month, you’re not alone. Researchers are finding that ADHD symptoms can fluctuate across the menstrual cycle, potentially leaving you undersupported in certain weeks.

Why Are My ADHD Symptoms Worse Right Before My Period?

Estradiol, a type of estrogen, appears to play an important role.

One 2024 qualitative study found that participants described worsening inattention, executive dysfunction, emotional dysregulation, and concentration difficulties in the luteal phase (the time between ovulation and your next period) and around menstruation. Participants also reported that their medication didn’t seem to work as well during this time.

A 2025 systematic review reached a similar conclusion: Sex hormones appear associated with changes in ADHD symptoms across the menstrual cycle. Research has linked lower estradiol with more severe ADHD symptoms.

The Connection Between Estrogen, Dopamine, and ADHD

Estrogen and progesterone interact with neurotransmitter systems including dopamine and noradrenaline, which help regulate attention, motivation, working memory, and executive functioning. These are also the neurotransmitters targeted by stimulant medications.
