New survey data reveals 79 percent of companies deploying agentic AI are suffering costly errors—and the traditional governance hacks IT teams rely on aren’t working.

Companies adopting artificial intelligence (AI) tools typically initially use them to automate repetitive, uninteresting, and low-value workplace tasks that employees previously performed. But a new study indicates many businesses have not only scaled their deployment to delegating critical decision-making roles to the tech, but have done so without sufficient controls to catch or quickly correct mistakes the applications make.

That gap between deployment and oversight was revealed by enterprise agent tool developer Kore.ai, which surveyed 400 business leaders at U.S. companies with 2,000 or more employees. Its........
