Tyrone Ward’s body was found by police officers at Mousehold Heath in Norwich, at around 4am on April 27, 2023.

He had been in regular contact with mental health services in the days before his death following two suicide attempts earlier that month on April 19 and April 21.

Despite this, Norfolk’s Crisis Resolution and Home Treatment Team (CRHTT) stood down a bed request on the eve of his death.
