Is Creatine the Next Big Thing for Brain Health?

Creatine monohydrate is one of the most extensively studied sports supplements, with strong evidence for improving performance during repeated, high-intensity exercise and supporting resistance-training adaptations. Now, it is being promoted well beyond the gym, for memory, mood, focus, and protection against cognitive decline.
The interest is understandable, but the science does not yet support treating creatine as a proven nootropic or brain-health supplement for healthy adults. The more accurate takeaway is that creatine is biologically plausible and promising in specific circumstances, but the human evidence remains early, mixed, and limited by small studies.
A 2026 narrative review by Darren G. Candow and colleagues, “Creatine Supplementation and the Brain: Have We Put the Cart Before the Horse?”, offers a useful, measured assessment of this question. The authors conclude that although creatine may support brain function under certain forms of metabolic stress, much more research is needed before broad cognitive or brain-health claims are justified.
The Biology Makes Sense
The adult brain represents roughly 2% of body mass but uses about 20% of the body’s energy at rest. Like muscle, it depends on ATP, the cell’s immediate energy source, to support its work.
Creatine helps buffer and regenerate ATP through the creatine-phosphocreatine system. In muscle, this system is especially important when energy demand rises rapidly during short, intense activity. In the brain, it may help cells manage fluctuating energy demands during situations such as sleep deprivation, mental fatigue, hypoxia, injury, or disease.
Preclinical research also suggests that creatine could affect processes relevant to brain health, including oxidative stress, neurotransmission, and cellular resilience.
