Susan Young, from Caister, died at the James Paget University Hospital in Gorleston at the age of 63, a day after being admitted after taking an overdose of prescription medication.

She was initially admitted to the emergency department at the hospital and monitored appropriately.

But after being transferred onto a ward, her monitoring did not continue, and staff were not warned of the risk of cardiac arrhythmias as a side........
