'Minor' car crash at hospital triggers string of blunders leading to woman's death
Susan Blake passed away from bronchopneumonia in January at the age of 73.
Her death came two-and-a-half weeks after she was injured in what was perceived to be a minor crash as she left an appointment at the Norfolk and Norwich University Hospital.

On December 19, Miss Blake had attended the N&N for a scan and as she was being driven home in a wheelchair accessible vehicle, it was t-boned at the emergency department roundabout.
The impact caused damage to the side and arm rest of her wheelchair and the court heard Miss Blake, who was non-verbal, screamed at the time of the impact.
Tarek Ahmed, an A&E consultant, came out of the department and carried out an assessment on her in the back of the car.
He told Norfolk Coroner's Court he had deemed her fit to go home without admission.
