Patient discharged after attempted fourth floor jump - and took own life hours later

Charlie Partridge was found dead following a deliberate act in August last year, leaving a scathing note describing feeling "let down" by "my own institution" - the NHS.

The 23-year-old had made an attempt to jump from the fourth floor of the Norfolk and Norwich University Hospital - tragically, the N&N staffer's own place of employment - but was restrained by security staff.

Despite the incident, mental health workers stuck to plans to discharge Charlie, who identified as non-binary.

Norfolk Coroner's Court heard Charlie had been admitted to hospital days earlier following an overdose and was being treated on a mental health ward.

But when staff made plans for discharge, Charlie was described as becoming withdrawn and refused to talk.

Later that evening, Charlie was found on the walkway ledge of the hospital's fourth floor and was restrained by staff after attempting to jump.

Nevertheless, the staff discharged Charlie........
