Mother of 'largely invisible' son to finally get answers three years after his death
Kurtis Ward, of Newmarket Street, Norwich, was found dead after taking his life at his home on July 2, 2022.
He was discovered by his mum, Rachel Spinney, who had previously raised concerns about the 21-year-old's care.
Among them include his case being closed by charity Voluntary Norfolk, despite it being referred to as "complex".
It was also revealed that Norfolk and Suffolk Foundation Trust (NSFT), which Mr Ward self-discharged from in 2021, did not alert her to three attempts to self-harm.

An independent safeguarding review was later commissioned, with the report highlighting Mr Ward as "largely invisible" within the system.
A central theme was that services focused on individual family members rather than recognising the combined pressures within the household.
Mr Ward shared caring responsibilities with his mum for his sibling, who has a rare chromosome disorder and learning disability.
However, the wider impact on the family, including Mr Ward's own wellbeing, was not consistently assessed.
