The response comes after an inquest heard concerns had been raised by both a teacher and a health visitor about squalid conditions at the family home in Lakenham and whether Draco’s mother was coping.

Through his barrister, Draco’s father David Chapman argued those warnings should have prompted a more urgent response from children’s social care.

The four-year-old died on April 2, 2019, after becoming trapped between two stacked stairgates while left alone in the front room of his home.

His mother Rachel Chapman had gone to a school meeting believing an older sibling was supervising him, but the sibling was unaware she had left the house.

During the inquest, much of the evidence focused on a referral made to children’s social care on March 26 - just days before Draco’s accident on April 2.
