A mother-of-two who overdosed on partner’s insulin 'failed by 111' inquest told
Rosie Cole, of Orchard Close, Norwich, was discovered at her home by her partner Lewis Ord after her family became concerned when they could not contact her.
An inquest heard she had contacted NHS 111 on the evening of August 27 last year seeking urgent help after suffering palpitations, incontinence and severe pain shortly after starting new medication for a rare genetic condition.
Although clinicians determined the 31-year-old required a home visit because she was unable to travel to an appointment, her case was closed after two follow-up telephone calls went unanswered.
It is not known whether Ms Cole had already died when those calls were made, but her body was found the following morning on August 28.

Her former partner, Lewis Ord, told a packed room at Norfolk Coroner’s Court that they had argued two days before.
As a result, he had taken their daughter to stay elsewhere, while Ms Cole’s elder child was already staying with his biological father.
The court heard Ms Cole had then sent Mr Ord seven messages asking whether their relationship was over, but he did not respond.
Following her death, toxicology findings revealed Ms Cole had an extremely high level of insulin in her body, causing fatal hypoglycaemia.
However, she was not a diabetic.
But Mr Ord, who has Type 1 diabetes, kept spare insulin pens at the property and police confirmed insulin containers were found during a search there.
Detective sergeant Emma Alston emphasised officers found no evidence of third-party involvement and treated the death as unexplained until the toxicology........
