Woman who died after 30 years of domestic abuse was failed by gaps in sharing details

An overview report by the Norfolk Community Safety Partnership, recently published as a Domestic Homicide Review (DHR), has outlined the tragic death of a woman who can only be identified as “Angela”.

The 52-year-old was found dead at her Norfolk home by her mother in May 2023 after hanging herself, with combined use of alcohol and diazepam also recorded as a cause of death.

Now the report has highlighted where lessons could be learnt from her death in the context of long‑term domestic abuse, mental ill‑health and alcohol misuse.

INCIDENTS FOR MORE THAN 30 YEARS

A panel was told Angela and her husband “Matthew” had been in contact with police over incidents of domestic abuse and violence for more than 30 years - although the timeline in the review runs from April 2020 to her death in 2023.

During that period, police officers were called to their address on multiple occasions, including an assault allegation against Matthew in April 2020 which led to his arrest and Angela being referred to a domestic abuse service.

The report goes on to describes Angela as a devoted mother to three adult children and a successful businesswoman who worked as a bookkeeper in the family firm and owned several companies.

Her children told the review she was a caring and nurturing parent, while family and friends remembered her as kind, happy, fun‑loving and always well‑dressed.
