Inmate's suicide days after release from prison prompts new safeguarding measures
The changes were revealed in a formal response by HMP Norwich to a prevention of future death (PFD) report issued by a coroner following an inquest into the death of Paul Thompson.
The 53-year-old took his own life after being struck by a freight train near Elmswell Railway Station in Suffolk on July 15, 2024 - just days after being released from custody.
An inquest heard Mr Thompson had repeatedly expressed suicidal thoughts while on remand at HMP Norwich.
Coroner Darren Stewart later issued a PFD report raising “serious concerns” about how prisoners with mental health needs were released from custody.
He found Mr Thompson had not been properly seen by mental health staff before being discharged from prison, despite being under ongoing care and having previously spoken about taking his own life by stepping in front of a train.
Probation services were also unaware of his release when he arrived at their office the following day.
In response, HMP Norwich governor - whose name was redacted in the published document - admitted a “significant mistake” had been made during the discharge process.
